Completing the Medical DOT Exam Report

The Medical DOT Exam Report allows a medical examiner to determine a driver's physical qualification to operate a commercial motor vehicle.

The Medical DOT Exam Report is not included in the default layout, but it is available to be added to a custom layout (see Defining Form Layouts).

1. On the Details tab, choose Actions»Medical DOT Exam Report. The Medical Examination Report is displayed with data transferred from the employee’s demographics.

2. Complete pages 1-3 by clicking on the appropriate check boxes or clicking in a text area and entering your response. Pages 4-8 contain instructional information, and page 9 contains the Medical Examiner’s Certificate that must be printed and completed by hand.

3. When the information is complete, use the toolbar options to print or email a copy of the report.

4. Click Save to save your entries and return to the previous screen, or click Delete to clear the form and return to the previous screen.
